Peace Trust a Non Governmental Organization was formed with the sole aim of serving the under privileged people through a HOLISTIC and meaningful approach. Peace Trust, a registered charitable trust has been working with the socially and economically poor, vulnerable people paying special attention to people to with lifestyle related and stigmatised diseases since 1997 in Tirunelveli District in Tamil Nadu, in South India.
Peace Trust has acted according to needs of the society at any given point of time like when the Tsunami struck the coastal districts of Tamil Nadu in December 2004 or engaging in Prevention Care and Support activities when the HIV situation in the district was growing to alarming propositions since September 2003 onwards. From a very humble beginning, Peace Trust is filling the void felt by the general community at large.

The inntegrated Counselling and Testing Centre is a gateway, a one stop shop for a host of related services in HIV/ AIDS Prevention, Care and Support. Treatment services like anti-retroviral Theraphy (ART), diagnosis and treatment of Tuberclosis, testing and treatment for Sexually Transmitted Infections, Reproductive diseases and any other health care requirements that a HIV patient may need are linked by the ICTCs with relevant services.
ICTCs provide psychological support and crisis counselling to people living with and directly affected by HIV. Successful ICTCs promote moving away from high risk behavior and towards sustained behavior changes. Ideally a health facility should have one integrated counselling and Testing Centre for all group of people. Peace Health Centre has been given the due honor of housing an Integrated Counselling and Testing Centre.
The main functions of our Peace ICTC include :
Counselling and Testing, Early Detection, Promotion of behavior change and reduction of vulnerability, linking people with other HIV prevention, Care and Treatment services.

The challenges related to health and diseases are one of the greatest challenges and they take different forms and intensity at different phases of life. While many diseases are curable, some are only manageable due to appropriate treatment facilities. Among the few diseases HIV/AIDS needs urgent intervention. The people with HIV/AIDS and those with most at risk specially Men indulging in sex with men, Trans Gender and Female Sex workers need an on going care and support especially when they are prone to such infections as STI and HIV. Tirunelveli District in the state of Tamil Nadu, South India is considered as one of the high HIV prevalent districts in the state by the National AIDS Control Organization. According to the Tamil Nadu HIV Sentinel Surveillance Report 2007, in Tirunelveli District, the HIV prevalence rate among ANC was 0.28%, among Female Sex workers at 0.8%, STI/HRB was 19.6% and among MSM AND FSW• was 5.6 % for whole of Tamil Nadu. Access to clinical care and support, treatment for STIs and supportive community care for the MSM AND FSW, FSW and Transgender community in Tirunelveli is non-existent. In Tirunelveli District according to AP AC mapping there are about 2205 MSM 2258 FSW and 17 TG’s.

Stake Holders Meeting
The MSM and FSW and Transgender groups are difficult to reach because of their invisibility and due to the stigma and denial attached to such behaviors. The sexual behavior of the MSM and FSW and TG groups is of serious concern, since usually they have multiple partners. Very few men and women in India are open about their sexual orientation. There are many men and women in India, especially in large cities and growing cities like Tirunelveli who engage in sex with other men before marriage. Many continue to have ex with men and women even after marriage. There are also men who have sex only with men and regard themselves as ‘gay’ or ‘homosexual’. There is an equally well-established transgender (hijra) community in Tirunelveli District. Sex between men is proscribed by the Indian Penal Code. Being ‘gay’ or ‘homosexual’ is widely regarded as deviant and morally reprehensible. The illegal status and public disapproval of their sexual orientation drives Men having Sex with Men in Tirunelveli”underground” for their sexual needs. This heightens their vulnerability to sexually transmitted infections (STIs) and HN/AIDS.

Goal of the Project:

“Reduce the vulnerability to STI/HIV infections and prevent further spread and re-infections among the MSMs and FSW Community attain total saturation and provide a continuum of care services to those indentified with HIV among the core groups and facilitate MSM and FSW community ownership in the eleven blocks of Tirunelveli District”.

RATIONALE OF THE PROJECT:

The challenge related to health and diseases are one of the greatest challenges and they take different forms and intensity at different phases of life. While many diseases are curable, some are only manageable due to appropriate treatment facility. The people with HIV/AIDs and those with most at risk specially when they are prone to such infections as ATI and HIV. Tirunelveli District in the state of Tamil Nadu, South India is considered as one of the high HIV prevalent districts in the state by the National AIDS Control Organization.

Target Area Profile:
Tirunelveli District in the state of Tamil Nadu,South India is one of the high STI/HIV Prevalent districts in the State. The key factor that increases the vulnerability of the people to HIV/AIDS is the high level of Migration. According to the census 2001 Report Tirunelveli has a population of 28,01,194 of which 12% are living below poverty line. Available reports suggest that every year nearly 6000 people migrate to Mumbai ,Chennai, kovai and other places in search of better employment opportunities. The failure of mom soon, lack of industries and the violence that prevailed between the different castes, from the early nineties forced more men folk to migrate. Agriculture is the main occupation of the people. Generally, these blocks have large area of barren land and lack industries. These blocks have more landless agricultural coolies, who depend on landlords for their daily wages and in most cases the land owners are from different castes .Presence of multiple castes in every location often results in communal disturbance and clashes.

Goal of the Project:

Reduce the vulnerability to STI,HIV/ AIDS and prevent further spread and promote safe sex practices among the in migrants community in Tirunelveli District.

Background of the Program:

In Patient Care
People living with HIV/AIDS (PLHA) require a range of support services including medical and non-medical care, institutional medical treatment and other supports depending on the stage and progression of the disease. The progression of the infection and consequent weakening of the immune system will result in PLHA being vulnerable for various opportunistic infections. The PLHA will require care and treatment for Opportunistic Infections (OI) and some of these illnesses may require in-patient care in a hospital or other centers that provide this facility. The CCCs are intended to function as a bridge between hospital and home care. They are playing a critical role in enabling PLHA to access ART and anti tuberculosis treatment as well as providing monitoring, follow-up, counseling support to those who are initiated on ART, positive prevention , drug adherence, nutrition counseling etc., Also CCCs are strengthening Community care and support programmes.

Goal of the Program:
To provide Quality Comprehensive Care through IP and OP Services and Enhance the quality of their lives by reducing their Morbidity to Opportunistic Infections to the PLHIV in the District of Tirunelveli.

Introduction

Friends of PLHIV Forum
The dedicated service is best for promotion and welfare of the people. In the view of eradicating the stigma, protect themseves and empower the infected to survive in the world with peace, Peace Trust has implementing the comprehensive prevention, care and support project in Tiruneveli corporation, Palayamkottai block. The project is being supported by BMZ/Malteser International and coordinated by Pro-vision since 2007. The programme was the coordinated effort of four implementing partners in Tirunelveli district with one specialization like lobbying and advocacy, Medical component, migration and industrial intervention.
There was tremendous changes had taken place in the last year 2009. The contributions of the project team members were much more higher in the development of the Project. Trainings for the PLHIV in special needs, registration of Nellai Sutram, get togethers and Nellai Sutram annual general body meeting was highlighted during the reporting period .

The goal of this programme is to enhance the standard of living of the PLHIV and of their corresponding kinsfolk through Home, Community and Institution Based care and thereafter preventing the spread of STI and HIV/AIDS in the Palayamkottai block, Tirunelveli Corporation and in the Kuruvikulam Blocks.

II. Care & Support
1. Life Skills Education sessions for Orphans and Vulnerable Children

The core life skills should be known by all the students to enrich their life in positive way . Based on the words , the project team were organized the LSE session to the OVC children in the targeted area to improve the skils in leadership, communication , differentiate the good and bad , handling emotional situation , HIV /AIDS , respecting the elders. CIA, CAA and OVC childrens were the participants of the LSE session . They were actively participated and learned with great interest .
2. Promotion of Self Care and Environmental Hygiene among PLHIV and their family members:
In the view of reducing the OI, the project team especially counsellors and nurses were trained the PLHIV and their family members on self care and environmental care during their home visits and support group meetings.
3. Promotion of awareness on bio medical waste disposals and proper funeral to the FAA :
Positive prevention is an important activities to prevent further spread of HIV . The family members are playing a vital role to provide the care to the PLHIV. They are also vulnerable to getting infection during extend the care of the PLHIV through improper handling of the PLHIV . In regards the nurses and counsellors were periodically provided the education to the Family members on bio medical waste management and proper funeral during the home visits and FAA support group meeting.
4. Impart knowledge to manage minor physical aliments at home
PLHIV are facing the minor aliments quite often in their day to day life. There is no possibility to go to hospital to access treatment at any time even for a small aliments. There is a waste of travel, spending money and physical disturbance. Hence the Nurses were provided the continuous education to the PLHIV to manage their physical aliments. The care givers were also extended their support to the PLHIV on OI management.
5. Psychological and Psycho social support to PLHIV
PLHIV are in the need of managing and overcoming their worries, suicidal tendencies and depression from their self and external factors. If they are not overcoming that, the problems are fully occupied them and even leads to mental problems and even death. In this regard , the project team were provided proper counseling and facilitation to the PLHIV to come out their stigma and making them to take decision by themselves regarding medicine and future directions . The team were promoted the PLHIV as a positive speaker.
6. Peer Care Givers:
To address the immediate needs and continue education in often to the door steps of the PLHIV, the peer care givers were actively working in the field for their peer promotion . They were regularly visited the PLHIV Home and trained the PLHIV as well as the family members on home based care, treatment seeking behavior ,importance of support group health centers and available government schemes . They were also motivated the peers to became as a positive speaker and role model to others. They were also shared their content of the visit to the PLHIV in their respective support group meeting for monitoring purposes.
7. Support Groups :
Support groups were functioned in their respective area with the active participation of their peers by themselves. They were well structural and making their groups to independently functioned with the support of the community and Philanthropist. During their support groups, they were shared their 1. Happiness 2. Sad happenings 3. Important events 4. Checking their treatment seeking 5.Problems identification 6.Plan to mobilise funds 7.Savings 8.Information regarding health centres 9.Jokes and news. Project team were streamlined their functions and facilitation work .
8. FAA /CAA Support Group Meeting :
In india , the support of the family is very important role in the development of the Individual. It decides the individuals health and mental stablity . Hence the project team were organized support group meeting for family members of the PLHIV . In this meeting , they were trained on HBC and care of PLHIV by the project team . And also shared the dos and donts among them for the betterment of PLHIV.
9. Formation and strengthening of Block level support Groups:
The block level support groups were functioned very effectively with the active participation of the support group leaders of the respective block . Here , they were shared the activities of the small support groups, steps to strengthening the activities and increase the activities of the Support Group . They are also taken the steps to strengthen Nellai sutram.
10. Strengthening Village level Friends of PLHIV Forums
Friends of forum is the best methodology to increase the involvement of the public towards the global cause . The forum was formed with the active participation of the community who are willingly come forward to support the PLHIV without discrimination. The community health workers were formed the friends of PLHIV forum in the target area.
11. Availability of adequate referral and linkages services to PLHIV Referrals to Govt. ART Centre :
In HIV AIDS field , ART centre has play a vital role to provide free treatment to the PLHIV and also offer the regular testing facility . They are also provided the treatment for OI and ART medicine to the PLHIV. Hence the PLHIV who are identified by us were immediately referred and registered at ART Centre. The important of the ART centre has emphasised by the project team during their home visit and support group meeting.
12. Referrals to Govt. Schemes & Widow pension:
The government has introduced lot of government schemes for the welfare of the people. Unfortunately, most of the people are not aware of the schemes and its procedures. Hence the project team, care givers and support group leaders were educated the PLHIV on government schemes and procedures. The eligible persons were referred and referred under various government schemes.
